The Arc Master Trust office will be closed on Thursday November 28th and Friday November 29th for the Thanksgiving holiday. We will only be processing disbursements once that week on Tuesday the 26th.

Enrollment Process

Trust I Enrollment Process

  • Complete the Trust I Joinder Agreement, available on our Documents & Resources page.
  • All donors should sign the Joinder Agreement.
  • Mail the Joinder Agreement, along with a check for the enrollment fee, to The Arc of Indiana Master Trust, PO Box 1547, Indianapolis, IN 46206.
  • Please make the check payable to The Arc of Indiana and note the beneficiary’s name on the memo line.
  • Please indicate if you have made arrangements to make payments on the enrollment fee or have made payment by credit card.
  • Once the completed Joinder Agreement is received and the enrollment fee has been paid, please allow 10 business days to process the enrollment. Once the enrollment is processed the donor(s) will receive a letter with the Trust I account number assigned to their trust and a copy of the completed trust documents. Eleven months after the date of the enrollment the donor(s) will receive an updated Joinder Agreement and statement for the renewal fee payment. Renewal fees are always due on the last day of the anniversary month in which the trust was opened.

Trust II Enrollment Process

  • Complete the Trust II Joinder Agreement, available on our Documents & Resources page.
  • The beneficiary, legal guardian, or parent may sign the Joinder Agreement.
  • If the beneficiary is a minor the parent(s) or legal guardian (appointed by the court) MUST sign the Joinder Agreement.
  • If there is a Guardian of Estate or Full Legal Guardian (Guardian of Person and Estate) that person or persons
    (if co-Guardians) must sign the Joinder Agreement.
  • If there is a Guardian of Person only that person or persons (if co-Guardians) MAY sign the Joinder Agreement.
  • The beneficiary may also sign in this case.
  • A copy of the guardianship papers (if appropriate) must be received in order to open the Trust II account.
  • If a lawsuit settlement will be used to fund the trust, a copy of the Court Order must be provided, along with the Joinder Agreement.
  • Please make the check to fund the trust payable to The Arc of Indiana. If the beneficiary’s name is not printed on the check, please note the beneficiary’s name on the memo line.
  • Mail the Joinder Agreement, check to fund the trust, copy of the guardianship papers (if appropriate), and copy of the Court Order (if appropriate) to The Arc of Indiana Master Trust, PO Box 1547, Indianapolis, IN 46206
  • Once all of the appropriate documents are received, please allow 10 business days to process the enrollment. Once the enrollment is processed the Key Person(s) will receive a copy of the Joinder Agreement, the Trust II account number assigned to the trust, and a Welcome Guide with information on how to access and use the trust. Whenever funds are added to a Trust II account, simply make the check payable to The Arc of Indiana, indicate the beneficiary’s name and/or their Trust II account number on the memo line, and mail the check to The Arc Master Trust office. If you would like confirmation that the addition has been made to the account please indicate that when you mail the check to us.
